Step 3 - Either mail the form and check to the address on the form
OR email the form to axozu25@gmail.com and we'll send you a paypal invoice to pay by email! We expect our turn-around time to be very quick and hope yours will be as well! Your registration is not complete until we receive your payment.
Details:
Tickets to the Silver Carnation Ball are $55 each.
MTM members get a $5 discount per ticket - all you have to do is pay your 2008-09 dues at the same time as you buy your tickets. Dues are $26 a year, unless you graduated (completed your undergraduate degree) in 2008, in which case your dues are free!
We are again offering the option of being a Lyre or a Hera patron. Lyre is $25 and Hera $50 (each is per person). Each patron will see his or name in our program, and receive special recognition at the event. Proceeds will benefit Zeta Upsilon and the Alpha Chi Omega Foundation. Thank you for your support!

Cancellation Policy: Full Refund of ticket price (less any paypal processing fees) if you cancel in writing (email to axozu25@gmail.com) by August 26, 2008; 50% refund of ticket prices (less any paypal processing fees) if you cancel in writing (email to axozu25@gmail.com) between August 27, 2008 and September 19, 2008. No refunds after September 19th. Dues are not refundable.
